Definition                                                                                                               
A questionnaire is divided into several sections.   Each section groups together questions of a similar nature.
Instructors can start to build an on-line questionnaire by clicking on this button.
Students can access to the on-line questionnaire through this link.
The identification of a questionnaire.  Students are required to input the Questionnaire ID before proceeding to the questionnaire.
The assigned identity by the instructors themselves in the system.  Students are asked to provide this information before accessing to the questionnaire.
There are several built-in questionnaires provided by the system.  Status "N" means that the current questionnaire is not a built-in questionnaire whereas "Y" means that it is a built-in one.  
A bundle of questions existing in the system.  There are over 800 selections which the instructors can choose from.
Instructors can insert notes or comments about the questionnaire in these fields. 
All built-in questions are grouped by sub-sections as well as sections.
This term refers to the ordering of sections within a questionnaire or the ordering of questions under each section.
A large box area where students can type in up to 255 characters answer for an open-ended question.
A box area where students can type in the answer for an open-ended question.
A type of answer option where students can click on the radio button next to their choice.  Only one answer is allowed for each question.
A type of answer option where students can check the box next to their choice. More than one answer can be selected.

   Answer You probably did not registered with the system properly.  Please try to register again.
triangle.gif (829 bytes) Question 2 Can I use OSTEI if I am not a UST staff?
     Answer OSTEI is open to the public.  Anyone with an e-mail address can register with the system.

   Answer No. OSTEI uses this piece of information to validate registered users to the system.

   Answer "Build Questionnaire" is a link for instructors to create questionnaire on-line whereas students will do the on-line questionnaire by clicking on the "Complete Questionnaire".

   Answer The system does not permit the users to modify the built-in questionnaire.

   Answer You can add up to 10 characters for the Questionnaire ID.

   Answer Login and update the questionnaire used in the evaluation.   Enter a new date for the End Date field.

   Answer Click on the 'Print' button from the toolbar of the browser.

  Answer There are no such button.  As long as you have saved your work and the preview of it is correct, you have completed the task.

   Answer The system will only allow the user to modify a section at a time.

   Answer The order number for the sections do not have to be in sequence.  The rule here is that small number goes in front of the larger ones.

   Answer No.  The user can add and delete built-in questions simultaneously but he/she must create a new question at a different time.
triangle.gif (829 bytes) Question 13 The system does not permit me to add a question to my questionnaire at the Add/Modify/Delete Questions page since the value under the Add column is N/A.
   Answer This is because the question already existed in the questionnaire.
triangle.gif (829 bytes) Question 14 I created a new question but when I click on 'Back' button, the question does not appear on Add/Modify/Delete Questions.
   Answer You must SAVE your work before you return to the Add/Modify/Delete Questions page.

     Answer Check whether you have correctly selected the format(A or B) at the page Your Question.  Make sure also that you have saved the information before exiting the page.
triangle.gif (829 bytes) Question 16 When I am creating a new question, I saved my work and then I removed my entries and enter new values for another new question. However, when I went back to the page Add/Modify/Delete Question, I only saw the last question saved. Why?
   Answer After saving your work, you must return to the page Add/Modify/Delete Question by clicking on the 'Back' button.  Check your saved work, then click on 'New Question' to create another new one.
