Question

Definition
Section	A questionnaire is divided into several sections.   Each section groups together questions of a similar nature.
Build Questionnaire	Instructors can start to build an on-line questionnaire by clicking on this button.
Complete Questionnaire	Students can access to the on-line questionnaire through this link.
Questionnaire ID	The identification of a questionnaire.  Students are required to input the Questionnaire ID before proceeding to the questionnaire.
UserName	The assigned identity by the instructors themselves in the system.  Students are asked to provide this information before accessing to the questionnaire.
Built-in Questionnaire	There are several built-in questionnaires provided by the system.  Status "N" means that the current questionnaire is not a built-in questionnaire whereas "Y" means that it is a built-in one.
Question Bank	A bundle of questions existing in the system.  There are over 800 selections which the instructors can choose from.
Description 1, 2 or 3	Instructors can insert notes or comments about the questionnaire in these fields.
Sub-section	All built-in questions are grouped by sub-sections as well as sections.
Order	This term refers to the ordering of sections within a questionnaire or the ordering of questions under each section.
Large Text Box	A large box area where students can type in up to 255 characters answer for an open-ended question.
Text Box	A box area where students can type in the answer for an open-ended question.
Radio Button	A type of answer option where students can click on the radio button next to their choice.  Only one answer is allowed for each question.
Checked Box	A type of answer option where students can check the box next to their choice. More than one answer can be selected.
Pulldown Option	A type of answer options where all answers are embedded in a pulldown list box.  Selection is made by highlighting the item.

Registration
	Question 1	I have not received an e-mail confirmation.
	Answer	You probably did not registered with the system properly.  Please try to register again.
	Question 2	Can I use OSTEI if I am not a UST staff?
	Answer	OSTEI is open to the public.  Anyone with an e-mail address can register with the system.
	Question 3	Can I refuse to accept the cookie when I log into the system?
	Answer	No. OSTEI uses this piece of information to validate registered users to the system.
Questionnaire
	Question 4	What is the difference between the buttons "Build Questionnaire" and "Complete Questionnaire"?
	Answer	"Build Questionnaire" is a link for instructors to create questionnaire on-line whereas students will do the on-line questionnaire by clicking on the "Complete Questionnaire".
	Question 5	I want to insert additional sections or questions to the built-in questionnaire.
	Answer	The system does not permit the users to modify the built-in questionnaire.
	Question 6	How many characters can I use for my Questionnaire ID?
	Answer	You can add up to 10 characters for the Questionnaire ID.
	Question 7	How can I extend the evaluation period?
	Answer	Login and update the questionnaire used in the evaluation.   Enter a new date for the End Date field.
	Question 8	How can I print out the report?
	Answer	Click on the 'Print' button from the toolbar of the browser.
	Question 9	I cannot find the 'Done' button.
	Answer	There are no such button.  As long as you have saved your work and the preview of it is correct, you have completed the task.
Section
	Question 10	How come I cannot select multiple sections at once?
	Answer	The system will only allow the user to modify a section at a time.
	Question 11	How do I order the sections?
	Answer	The order number for the sections do not have to be in sequence.  The rule here is that small number goes in front of the larger ones.
Question
	Question 12	Can I add, delete and create new questions at the same time.
	Answer	No.  The user can add and delete built-in questions simultaneously but he/she must create a new question at a different time.
	Question 13	The system does not permit me to add a question to my questionnaire at the Add/Modify/Delete Questions page since the value under the Add column is N/A.
	Answer	This is because the question already existed in the questionnaire.
	Question 14	I created a new question but when I click on 'Back' button, the question does not appear on Add/Modify/Delete Questions.
	Answer	You must SAVE your work before you return to the Add/Modify/Delete Questions page.
	Question 15	The answer options are not showing up when I preview the questionnaire.
	Answer	Check whether you have correctly selected the format(A or B) at the page Your Question.  Make sure also that you have saved the information before exiting the page.
	Question 16	When I am creating a new question, I saved my work and then I removed my entries and enter new values for another new question. However, when I went back to the page Add/Modify/Delete Question, I only saw the last question saved. Why?
	Answer	After saving your work, you must return to the page Add/Modify/Delete Question by clicking on the 'Back' button.  Check your saved work, then click on 'New Question' to create another new one.
